Care Coordinator Job Summary

Brook Street Social Care NMR is seeking a skilled Care Coordinator to join our team in the West Midlands area. This role involves implementing new care packages, allocating staff, and ensuring high-quality care delivery.

Key Responsibilities:

  • Implementing new care packages for individuals with medium to complex care needs.
  • Allocating staff to match their expertise with client needs.
  • Maintaining contact with service users and updating care plans regularly.
  • Preparing monthly reports for the Clinical Commissioning Group (CCG).

Requirements:

  • Proven experience in a similar role.
  • Relevant care qualification (level 3 in health and social care) or willingness to work towards this.
  • Outstanding communication skills, attention to detail, and strong organisation abilities.
  • Full UK driving licence, access to a vehicle, and flexibility to travel throughout the West Midlands.
