Job Title: Home Care Specialist Lead

Location: West Midlands based, with flexibility to work between central Wolverhampton Office and in the field.

Salary: £25,000 - £30,000 per annum, depending on experience.

Job Overview

We are seeking an experienced Home Care Specialist Lead to join our team at Brook Street Social Care. As a key member of our care team, you will play a vital role in delivering high-quality care packages to individuals in their own homes.

You will be responsible for implementing new care packages, allocating care staff effectively, and matching their expertise with the needs of our service users. You will also maintain contact with service users, ensure person-centred care is delivered, and update care plans regularly.

Key Responsibilities
  • Implementing new care packages and allocating care staff effectively.
  • Maintaining contact with service users in person and over the phone.
  • Ensuring person-centred care is delivered and care plans are updated regularly.
  • Meeting with clinicians and external providers to review care packages.
  • Preparing and sending monthly reports to the Clinical Commissioning Group (CCG).
Requirements

To be successful in this role, you will need:

  • A relevant care qualification (level 3 in health and social care or willing to work towards this).
  • A full UK driving licence, access to your own vehicle, and flexibility to visit clients and professionals throughout the West Midlands.
