Care Management Coordinator

Delivering High-Quality Care Services in the West Midlands
We are seeking a highly skilled Field Care Supervisor to join our team at Brook Street UK. This role is ideal for individuals with a passion for delivering person-centred care and ensuring the independence of clients with medium to complex needs.
The successful candidate will be responsible for implementing care packages, allocating staff, and maintaining contact with service users. They will also be required to meet with clinicians and external providers to review care packages and ensure delivery in line with company and CQC standards.
About the Role
- Implement new care packages for service users, tailoring support to their individual needs
- Effectively allocate care/support staff, ensuring they have the necessary expertise to deliver high-quality care
- Maintain regular contact with service users, providing ongoing support and reassurance
- Ensure person-centred care is delivered, with care plans updated regularly to reflect changing needs
- Meet with clinicians and external providers to review care packages and ensure compliance with industry standards
Requirements and Qualifications
- Relevant care qualification (level 3 in health and social care or willing to work towards this)
- Full UK driving licence and access to own vehicle for visiting clients and professionals throughout the West Midlands
- Generous annual salary ranging from £25,000 to £30,000 depending on experience

Salary Details
We offer a competitive salary range of £28,000 per annum, based on experience, which reflects the skills and qualifications required for this role. The salary includes a generous annual increase, allowing you to grow professionally and financially with our organisation.
