About the Role

We are seeking a skilled and compassionate Community Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Community Nurse, you will be responsible for providing high-quality care to patients in their own homes, care homes, and clinics.

Key Responsibilities

  • Assess, plan, implement, and evaluate patient care, developing individualized care plans using clinical reasoning skills.
  • Conduct holistic assessments, communicating complex information to patients and carers, and modifying care plans as needed.
  • Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care is coordinated.
  • Participate in multi-disciplinary meetings, contributing to the delivery of high standards of healthcare.

Requirements

  • Diploma or 1st-level degree in Nursing, RN qualification, and current NMC registration.
  • Experience of working within a clinical setting, with a strong desire to further develop community nursing skills and experience.
  • Ability to provide high standards of care, highly motivated, and able to work flexibly to accommodate patient/service needs.
