About the Role

We are seeking a skilled and compassionate Community Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Community Nurse, you will play a vital role in delivering high-quality patient care in the community, working closely with patients, families, and healthcare professionals to provide holistic and person-centered care.

Key Responsibilities

  • Assess, plan, implement, and evaluate patient care, using clinical reasoning skills to develop individualized care plans.
  • Conduct holistic assessments, communicating complex information to patients and carers, and modifying care plans as needed.
  • Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care is coordinated.
  • Participate in multi-disciplinary meetings, contributing to the development of care plans and ensuring seamless service delivery.
  • Work collaboratively with the team to achieve service targets and patient outcomes.

Requirements

  • Diploma or 1st-level degree in Nursing, with current NMC registration.
  • Experience of working within a clinical setting, with a strong understanding of community care principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to provide high standards of care, with a focus on patient-centered and holistic care.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage a caseload effectively.
