Job Title: Care Coordinator for Complex Needs

Job Summary: We are seeking a highly skilled Registered Nurse to join our team as a Care Coordinator for Complex Needs. As a Care Coordinator, you will work with clients across Nottinghamshire, providing ongoing visits to create care and support plans, undertaking annual support reviews, risk assessments, inductions, supervisions, staff training, and other visits necessary to keep the person safe.

Key Responsibilities:

* Develop and implement care plans for clients with complex needs
* Provide ongoing support and supervision to clients and their families
* Collaborate with other healthcare professionals to ensure comprehensive care
* Conduct risk assessments and develop strategies to mitigate risks
* Provide training and education to staff on complex care

Requirements:

* Registered Nurse (RGN) with a strong background in community care
* Experience in developing care plans and providing ongoing support
* Strong clinical skills and ability to work independently
* Full UK driving licence and car owner
* Ability to work flexible hours and manage own diary
