Job Overview

The Frailty Virtual Ward is a pioneering initiative within York and Scarborough Teaching Hospitals NHS Foundation Trust, offering a unique opportunity for a Consultant Respiratory Medicine Specialist to join a multidisciplinary team in providing acute care to patients within their own home environment. This role is designed to support early hospital discharge and admission avoidance, while contributing to the development of this innovative service.

Main Duties and Responsibilities
  • Assess patients in their own home environment, working collaboratively with the multidisciplinary team to devise holistic management plans.
  • Triage referrals, conduct comprehensive geriatric assessments, perform diagnostics, create treatment plans, and refer to specialty colleagues as necessary.
  • Develop and implement quality improvement projects to further enhance the Frailty Virtual Ward service.

Person Specification
  • Essential criteria:
    • Holds a full UK driving licence and has access to their own vehicle.
    • Able to communicate effectively in written and spoken English.
    • Experience of working in multidisciplinary teams.
    • Able to establish good working relations with patients, carers, and multidisciplinary colleagues.
  • Desirable criteria:
    • Understanding of the responsibilities of being an employee of the NHS.
    • Competence in basic clinical procedures.
    • Able to prioritise clinical need, time, and workload.
Qualifications and Experience
  • Essential criteria:
    • Has achieved MBBS or equivalent medical qualification.
    • ALS training or equivalent.
    • Evidence of NHS employment in a training or service post.
    • Satisfactory completion of Foundation Training or equivalent.
    • Full registration with the GMC at time of starting the post.
