Job Overview

The Frailty Virtual Ward is a pioneering initiative within York and Scarborough Teaching Hospitals NHS Foundation Trust, offering a unique opportunity for a skilled Consultant Respiratory Medicine Specialist to join a multidisciplinary team. This innovative service provides acute care to patients within their own home environment, supporting early hospital discharge and admission avoidance.

Main Duties and Responsibilities
  • Assess patients in their own home environment, working collaboratively within the multidisciplinary team to devise holistic management plans.
  • Triage referrals, conduct comprehensive geriatric assessments, perform diagnostics, create treatment plans, and refer to specialty colleagues as appropriate.
  • Develop and implement quality improvement projects to further enhance the Frailty Virtual Ward service.

Person Specification
  • Essential criteria:
    • Holds full UK driving licence and has access to own car.
    • Able to communicate effectively in written and spoken English.
    • Experience of working in multidisciplinary teams.
    • Able to establish good working relations with patients, carers, and multidisciplinary colleagues.
  • Desirable criteria:
    • Understanding of the responsibilities of being an employee of the NHS.
    • Competence in basic clinical procedures.
    • Able to prioritise clinical need, time, and workload.
Qualifications and Experience
  • Essential criteria:
    • Has achieved MBBS or equivalent medical qualification.
    • ALS training or equivalent.
    • Evidence of NHS employment in a training or service post.
    • Satisfactory completion of Foundation Training or equivalent.
    • Full registration with the GMC at time of starting the post.
