Job Overview

The Frailty Virtual Ward is a pioneering initiative within York and Scarborough Teaching Hospitals NHS Foundation Trust, led by a Geriatrician and comprising a multidisciplinary team. This innovative service aims to provide acute care to patients in their own homes, facilitating early hospital discharge and admission avoidance.

Main Responsibilities

The successful candidate will be responsible for assessing patients in their home environment, collaborating with the multidisciplinary team to devise holistic management plans, and performing diagnostics as required. Day-to-day duties will include triaging referrals, creating treatment plans, and referring to specialty colleagues as necessary.

Person Specification

The ideal candidate will possess a strong background in respiratory medicine, with experience in working with frail patients. They will be able to communicate effectively with patients, carers, and multidisciplinary colleagues, and demonstrate a keen interest in frailty and geriatric care. A full UK driving licence and access to a vehicle are essential for this role.
