Care Coordinator Job Description

Proline Care Recruitment Limited is seeking a highly skilled and experienced Care Coordinator to join our team. As a Care Coordinator, you will play a key role in ensuring the smooth running of visits to our clients, working closely with senior managers to achieve this goal.

Key Responsibilities:

  • Accepting care packages and assigning care workers to service users in an appropriate manner
  • Producing care workers rota and managing the roster software (training provided)
  • Using electronic monitoring software to monitor care workers attendance and timeliness for visits
  • Arranging care reviews and risk assessments
  • Conducting telephone monitoring and ensuring service quality
  • Maintaining high quality standards, systems and procedures set by the company and in line with CQC Compliance and Health & Social Care Act 2008 and contract compliance as applicable
  • Taking instructions from the Home-care Manager and Directors

Requirements:

  • Recent experience within a care setting is required for this role
  • You need to have good computer skills
