About the Role

We are seeking a highly skilled and experienced Perinatal Mental Health Specialist to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a Care Coordinator, you will play a vital role in providing comprehensive mental health assessment and care coordination for service users with perinatal mental health needs.

Key Responsibilities
  • Undertake comprehensive mental health assessments for service users with perinatal mental health needs, using standardized assessment tools and frameworks.
  • Develop and deliver complex treatment programmes, including strategies to manage risk for service users with complex needs and carers.
  • Act as a care coordinator for service users with the most complex needs, providing defined interventions and support.
  • Deliver a range of defined activities and interventions to improve carers' ability to support service users and enable them in their relationship with the service.
  • Develop and maintain good partnership working with other services throughout all treatment episodes, including regular liaison with Primary Health Care Teams, inpatient and intensive services, day services, and voluntary sector organizations.
  • Facilitate the development of complex comprehensive crisis plans, rapid access plans, and advance statements, involving other agencies as necessary.
  • Build and lead hope-inspiring relationships with service users, acknowledging their personal journey and focusing on strengths and aspirations to enable the creation of meaningful personal recovery.
  • Maintain own workload and plan the workload of others, ensuring effective time management and utilization of electronic resources.
  • Protect individuals from abuse and harm, working with others to plan, develop, and review protection plans, and coordinating and undertaking protective interventions as necessary.
  • Collaborate with service users and carers to develop skills to manage their own health, promoting and using approaches that are affirming, build on strengths, identify past positive experiences and success, and use small steps to move towards the person's goals.
  • Undertake supervision for other colleagues within the team, covering for the manager and colleagues as necessary.
  • Maintain the single health and social care record, ensuring both paper and electronic records are kept up to date in accordance with professional and organizational requirements.
  • Report and record within agreed timeframes, all activity relating to information reporting and performance.
  • Participate in management, caseload, and clinical supervision in accordance with trust policies and procedures.
  • Provide mentoring and training for others in relevant practice areas, according to professional requirements, taking a collaborative approach to practice development and evidence-based care.
Person Specification
  • Registered as an Occupational Therapist, Social Worker, or Registered Mental Health Nurse (parts 3 or 13 of NMC register).
  • Formal post-graduate diploma level qualification in a relevant field.
  • Significant post-qualifying clinical experience obtained in a variety of settings.
  • Experience of working with women experiencing perinatal mental health needs/knowledge of demographic risk factors for this client group.
  • Clear record of ongoing continuing professional development.
  • Demonstrates experience as a Care Coordinator or equivalent role of complex cases.
  • Demonstrates a detailed understanding in the theory and practice of Recovery Principles.
