About the Role

We are seeking a highly skilled and experienced Band 6 Specialist Mental Health Practitioner to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust.

Key Responsibilities
  • Deliver comprehensive mental health assessments for service users with severe and enduring mental health needs.
  • Develop and deliver complex treatment programmes using evidence-based practice.
  • Act as care coordinator for service users with complex needs and provide defined interventions to individuals on other workers' caseloads.
  • Lead the CPA process and work collaboratively with other professionals to co-ordinate interventions.
  • Provide therapeutic interventions, including individual or group therapy, and medication management.
  • Develop and maintain good partnership working with other services, including primary health care teams, inpatient and intensive services, and voluntary sector organisations.
  • Facilitate the development of complex crisis plans and advance statements, involving other agencies as necessary.
  • Build and lead hope-inspiring relationships with service users, focusing on strengths and aspirations to create meaningful personal recovery plans.
  • Maintain own workload and plan the workload of others, ensuring effective time management and utilisation of electronic resources.
  • Protect individuals from abuse and harm, working with others to develop and review protection plans and ensuring appropriate information sharing.
  • Co-ordinate and undertake protective interventions as outlined in protection plans.
Person Specification
  • Current registration with the NMC or HCPC or SWE and commitment to CPD.
  • Diploma level/Degree in a relevant health profession.
  • Ability to deliver a range of key therapeutic interventions, including CBT, psychosocial interventions, and family interventions.
  • UK driving licence with business insurance.
