About the Role

We are seeking a highly skilled and dedicated Mental Health Specialist Practitioner to join our team at Avon and Wiltshire Mental Health Partnership NHS Trust. As a key member of our community-based care team, you will play a vital role in delivering high-quality care to individuals with severe and enduring mental health needs.

Key Responsibilities
  • Deliver comprehensive mental health assessments and develop personalized care plans in collaboration with service users, carers, and other healthcare professionals.
  • Provide therapeutic interventions, including CBT, psychosocial, and family interventions, to support service users in achieving their recovery goals.
  • Work closely with other healthcare professionals to coordinate care and ensure seamless transitions between services.
  • Develop and maintain good partnership working with other services, including primary care, inpatient services, and voluntary sector organizations.
  • Facilitate the development of complex crisis plans, rapid access plans, and advance statements in collaboration with service users and carers.
Requirements
  • Current registration with the NMC or HCPC and commitment to ongoing professional development.
  • Degree in a relevant health profession, such as nursing or psychology.
  • Ability to deliver a range of therapeutic interventions, including CBT and psychosocial approaches.
  • UK driving license with business insurance.
