About the Role

We are seeking a highly skilled and dedicated Mental Health Specialist Practitioner to join our team at Avon and Wiltshire Mental Health Partnership NHS Trust. As a key member of our community mental health team, you will play a vital role in delivering high-quality care to individuals with complex mental health needs.

Key Responsibilities
  • Deliver comprehensive mental health assessments and develop personalized care plans for service users.
  • Provide therapeutic interventions and support to individuals with severe and enduring mental health needs.
  • Work collaboratively with other professionals to coordinate care and ensure seamless transitions between services.
  • Develop and maintain good partnership working with other services, including primary health care teams, inpatient services, and voluntary sector organizations.
  • Facilitate the development of complex crisis plans and advance statements, involving other agencies as necessary.
  • Build and maintain hope-inspiring relationships with service users, focusing on their strengths and aspirations to create meaningful personal recovery plans.
  • Manage and prioritize workload effectively, utilizing electronic resources to optimize time and efficiency.
  • Protect individuals from abuse and harm, working with others to develop and review protection plans and ensuring appropriate information sharing.
  • Participate in on-call rotas and local arrangements to ensure consistent care across the local geography.
Person Specification
  • Current registration with the NMC or HCPC or SWE and commitment to CPD.
  • Diploma level/Degree in a relevant health profession.
  • Ability to deliver a range of key therapeutic interventions, including CBT, psychosocial interventions, and family interventions.
  • UK driving license with business insurance.
