Job Summary

We are seeking a highly skilled and motivated Mental Health Specialist Practitioner to join our team at Avon and Wiltshire Mental Health Partnership NHS Trust. As a key member of our community mental health team, you will be responsible for delivering high-quality care and support to individuals with complex mental health needs.

Main Responsibilities
  • Deliver comprehensive mental health assessments and develop personalized care plans for service users.
  • Provide therapeutic interventions and support to individuals with severe and enduring mental health needs.
  • Work collaboratively with other professionals to coordinate care and services, ensuring a seamless and effective treatment pathway.
  • Develop and maintain good relationships with service users, carers, and other stakeholders, promoting a person-centered approach to care.
  • Contribute to the development of the team and the organization, sharing knowledge and expertise to improve practice and outcomes.

Person Specification
  • Current registration with the NMC or HCPC or SWE and commitment to CPD.
  • Diploma level/Degree in a relevant health profession.
  • Ability to deliver a range of therapeutic interventions, including CBT, psychosocial interventions, and family interventions.
  • UK driving license with business insurance.
