About the Role

We are seeking a highly skilled and experienced Mental Health Specialist Practitioner to join our Community Mental Health Team as a Band 6 Specialist Recovery Practitioner. This is an exciting opportunity for a dedicated professional to work in a focused way developing skills for an identified group of individuals on a psychotic pathway, improving the outcomes and experience for our service users and their carers.

Key Responsibilities
  • Care coordination of a complex caseload of service users with psychotic illnesses, including those with co-existing substance misuse issues.
  • Leading the Care Programme Approach (CPA) process, ensuring comprehensive and coordinated care planning.
  • Multi-agency working, collaborating with other professionals to co-ordinate interventions and achieve positive outcomes.
  • Risk management, working with service users and carers to develop and implement risk management plans.
  • Administration of medications, ensuring safe and effective medication management.
  • Fulfilling the Community Mental Health Team (CMHT) duty worker role on a rolling rota, providing 24/7 support to service users.
Person Specification
  • Current registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC) and commitment to Continuing Professional Development (CPD).
  • Diploma level/Degree in a relevant health profession.
  • Ability to deliver a range of therapeutic interventions, including Cognitive Behavioural Therapy (CBT), Psychosocial interventions, and Family Interventions.
  • UK driving licence with business insurance.
