About the Role

We are seeking a highly skilled and dedicated Mental Health Specialist Practitioner to join our team at Avon and Wiltshire Mental Health Partnership NHS Trust. As a key member of our community-based care team, you will be responsible for delivering high-quality care and support to individuals with severe and enduring mental health needs.

Key Responsibilities

  • Undertake comprehensive mental health assessments and develop personalized care plans to meet the unique needs of each service user.
  • Deliver a range of therapeutic interventions, including CBT, psychosocial interventions, and family therapy, to promote recovery and well-being.
  • Work collaboratively with other healthcare professionals, including primary care teams, inpatient services, and voluntary sector organizations, to ensure seamless care coordination.
  • Develop and maintain good relationships with service users, carers, and families, to promote empowerment and involvement in care planning.
  • Participate in on-call rotas and contribute to the development of local safeguarding policies and procedures.

Person Specification

  • Current registration with the NMC or HCPC, with a commitment to ongoing professional development.
  • Degree in a relevant health profession, such as nursing or psychology.
  • Ability to deliver a range of therapeutic interventions, including CBT and psychosocial interventions.
  • UK driving license with business insurance.
