About the Role

We are seeking a highly skilled and compassionate Health Care Support Worker to join our Community Health Team at Wiltshire Health and Care LLP. As a key member of our team, you will play a vital role in providing high-quality patient care and support in the community.

Key Responsibilities
  • Provide personalized care and support to patients in their homes and community settings.
  • Work collaboratively with our multidisciplinary team to deliver effective patient care and outcomes.
  • Develop and maintain strong relationships with patients, families, and carers to ensure seamless care coordination.
  • Contribute to the development and implementation of patient care plans, ensuring individualized and person-centered approaches.
  • Participate in ongoing training and professional development to enhance skills and knowledge in community health care.
Requirements
  • Good general education and relevant qualifications in health care or a willingness to achieve.
  • Experience of working under distanced supervision and supporting other workers.
  • Access to a vehicle for business purposes and a full and valid driving license.
  • Ability to work flexibly, including weekends and late shifts, to meet patient care needs.
