Job Title: Team Leader Support Worker

Job Summary:

We are seeking a highly skilled and experienced Team Leader Support Worker to join our team at Libertatem Healthcare Group. As a Team Leader Support Worker, you will be responsible for providing high-quality care and support to our clients, as well as managing a team of support workers to ensure the delivery of exceptional care services.

Key Responsibilities:

1. Leadership and Team Management:

• Provide leadership and guidance to the support team to ensure the delivery of high-quality care services.

• Motivate and support team members to achieve their full potential.

• Manage team rotas and annual leave to ensure adequate staffing levels.

• Conduct regular supervision and appraisal meetings with team members.

2. Care and Support:

• Provide high-quality care and support to clients, as defined in their care and support plans.

• Assist clients with daily living tasks, such as personal care, meal preparation, and household chores.

• Promote independence and self-management among clients.

• Administer medication as designated in the care and support plan.

3. Communication and Collaboration:

• Communicate effectively with clients, families, and other healthcare professionals to ensure seamless care delivery.

• Work collaboratively with the healthcare coordinator and clinical lead nurse to ensure client needs are met.

• Report any concerns or incidents to the office team, following company procedure.

4. Health and Safety:

• Ensure a safe and healthy environment for clients and staff.

• Identify and report any hazards or potential hazards that impact on the safe delivery of care.

5. Data Protection and Confidentiality:

• Respect the confidentiality of client information and maintain confidentiality at all times.

• Ensure that client data is processed and stored in accordance with company policies and procedures.

6. Training and Development:

• Complete mandatory and client-specific training to achieve the specified level of competency.

• Participate in regular supervision and appraisal meetings to discuss training needs and goals.

Requirements:

• Experience in a care or support role, with a focus on leadership and team management.

• Strong communication and interpersonal skills.

• Ability to work collaboratively with clients, families, and healthcare professionals.

• Strong problem-solving and decision-making skills.

• Ability to maintain confidentiality and handle sensitive information.
