Job Summary

The Midlands Partnership NHS Foundation Trust is seeking a skilled Community Mental Health Nurse to join our team in Telford. As a key member of our Urgent Care Pathway, you will work closely with a multidisciplinary team to provide mental health assessment, treatment, and support to individuals in Telford and Wrekin.

Main Responsibilities

As a Community Mental Health Nurse, you will be responsible for:

  • Providing specialist assessment and care planning in conjunction with service users and carers
  • Evaluating treatment plans and undertaking home treatment
  • Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
  • Coordinating the work of the team
