Practitioner for Adults with Learning Disabilities

We are looking for a compassionate and motivated individual to join our team as a Support Practitioner in Perth & Kinross. This role offers a unique opportunity to work with adults who have complex needs, providing personalized support to promote their well-being and independence.
As a Support Practitioner, you will be part of a dynamic team that delivers services for individuals with learning disabilities and autism. You will work closely with colleagues, family members, and other professionals to develop tailored support plans, addressing each service user's specific needs and goals.
Key Responsibilities:
- Deliver person-centered support to adults with complex needs
