Community Mental Health Practitioner

We are seeking a Community Mental Health Practitioner to join our team at Invicta Health in partnership with Kent and Medway NHS and Social Care Partnership Trust (KMPT). As part of the Mental Health Together initiative, this role will play a vital part in delivering high-quality care to individuals with mental health needs.
About the RoleThe successful candidate will be a Registered Mental Health Nurse or Occupational Therapist responsible for assessing, planning, implementing, and evaluating care for patients allocated to them. They will work closely with the Multi-Disciplinary Team (MDT), Crisis Resolution Home Treatment (CRHT), Acute Inpatients Services, other community teams, Primary Care, and the Voluntary Sector to provide support and treatment to service users.
Main Responsibilities- Foster recovery through empowerment of service users and carers by using a strengths-based approach.
- Offer a range of evidence-based interventions and risk management approaches enabling service users and carers to reduce and manage symptoms and high-risk behavior.
- Be proactive, innovative, and creative in delivering care, enabling the facilitation of service users' recovery.
We are offering a competitive salary of £38,890 - £44,886 per annum, depending on experience. The role also offers opportunities for learning and development, as well as a range of benefits including a staff forum, progression opportunities, and a company car lease scheme.
