About the Role

We are seeking a dedicated and compassionate Mental Health Support Worker to join our team at Gray Healthcare. As a Support Worker, you will play a vital role in supporting an individual with complex needs, disabilities, and long-term conditions in their own home.

Key Responsibilities

  • Provide 2:1 support to the individual, promoting independence and empowering them to make informed decisions.
  • Assist with daily living tasks, such as cooking, cleaning, and personal care.
  • Encourage the individual to engage in activities they enjoy, such as swimming, outdoor walks, and bingo.
  • Support the individual in managing their medication and implementing their care plan.
  • Work collaboratively with the multidisciplinary team to ensure the individual's needs are met.

Requirements

  • Full UK Driving License (essential)
  • Social care experience, ideally within mental health and with experience of supporting individuals with Schizophrenia.
  • Experience providing encouragement to individuals with complex health needs and Learning Disabilities.
  • Experience supporting individuals with challenging behaviors.
  • Ability to implement detailed care plans and risk assessments.
  • Current Right to Work in the UK and at least 6 months of employment in the UK.
