About the Role:

Ambitions Personnel is seeking a dedicated and compassionate Housing and Health Specialist to join our team. As a Complex Needs Practitioner, you will play a vital role in supporting young people with complex needs, promoting independence, and working closely with management to develop and deliver effective support plans.

Key Responsibilities:

  • Plan and Review Support Plans: Develop and implement personalized support plans to meet the unique needs of each young person, working closely with management to ensure effective delivery.
  • Complete and Update Risk Assessments: Conduct thorough risk assessments to identify potential challenges and develop strategies to mitigate them, ensuring the safety and well-being of young people.
  • Develop Alternative Support Strategies: Collaborate with the management team to develop and deliver alternative support methods for young people who require additional support, ensuring that their needs are met and their independence is promoted.
  • Develop Tools and Resources: Create and maintain tools and resources to support young people in managing and changing their behavior, working closely with management to ensure their effectiveness.

Requirements:

  • Full UK Driving Licence: Possess a valid full UK driving licence to enable travel to various locations.
  • DBS Check: Complete a DBS check upon hiring to ensure a safe and secure working environment.
