About the Role

As a Support Service Coordinator, you will be responsible for coordinating the delivery of support services to service users, ensuring that their needs are met and their quality of life is enhanced. You will work closely with multidisciplinary teams to develop and implement personalized care plans.

Key Responsibilities
  • Coordinate the delivery of support services to service users, including medication management and personal care.
  • Develop and maintain strong relationships with service users, their families, and carers.
  • Collaborate with multidisciplinary teams to ensure seamless transitions between care settings.
Requirements
  • A relevant qualification in healthcare or social care (e.g., NVQ 3 or equivalent).
  • Experience of working in a care setting, preferably with adults with complex needs.
  • Excellent communication and organizational skills.
  • A commitment to delivering person-centered care and promoting independence.
