Job SummaryWe are seeking a skilled Care Manager to join our team. As a key member of our staff, you will work closely with families and youth to provide support and guidance, helping them navigate the complexities of mental health challenges.
Key Responsibilities- Develop and implement personalized care plans to meet the unique needs of each family and youth.
- Build and maintain strong relationships with families, youth, and community partners to ensure seamless support and coordination.
- Provide education and resources to families and youth on mental health topics, self-care, and coping strategies.
- Collaborate with our multidisciplinary team to ensure comprehensive and coordinated care.
- Document progress and outcomes, using data to inform care decisions and improve services.
- Bachelor's degree in social work, psychology, education, guidance, human services, or a related field.
- At least two years of experience working with youth facing mental health challenges, demonstrating a strong work ethic and performance record.
- Ability to use your own vehicle for local travel, as needed.
- A competitive hourly rate of $84, with regular wage increases and bonuses.
