Clinical Pharmacist

Mid Norfolk Primary Care Network (PCN) is seeking an experienced clinical pharmacist to join their team on a full-time basis. The successful candidate will be responsible for managing the prescription request workflow, conducting complex face-to-face medication reviews, and developing and managing medicines optimization services within the network practices.
Main Responsibilities- Manage the prescription request workflow to ensure efficient and effective medication management.
- Conduct complex face-to-face medication reviews with a focus on polypharmacy, deprescribing, and opiate prescribing.
- Develop and manage medicines optimization services within the network practices to improve patient outcomes.
- Provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship.
- Support the management of patients with long-term conditions and undertake clinical medication reviews to produce recommendations for senior clinical pharmacists, nurses, or GPs.
- Work with care home staff to improve the safety of medicine ordering and administration.
The ideal candidate will be a qualified pharmacist with strong experience in primary care. Essential qualifications include a minimum of 2 years post-qualifying experience, completion of an undergraduate degree in pharmacy, and working towards an independent prescribing qualification.
The successful candidate will possess excellent interpersonal, influencing, and negotiating skills, as well as excellent written and verbal communication skills. They will also demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of patients.
About UsMid Norfolk PCN consists of six GP surgeries working closely together as a collaborative group. The successful candidate will be part of this team and will work closely with other healthcare professionals to improve patient outcomes.
