Clinical Pharmacist

Mid Norfolk Primary Care Network (PCN) is seeking an experienced clinical pharmacist to join their team on a full-time basis. The successful candidate will be responsible for managing the prescription request workflow, conducting complex face-to-face medication reviews, and developing medicines optimisation services within the network practices.
Main Responsibilities- Manage the prescription request workflow.
- Conduct medication reviews with a focus on polypharmacy, deprescribing, and opiate prescribing.
- Develop and manage medicines optimisation services.
- Provide leadership on quality improvement and clinical audit.
- Support the management of patients with long-term conditions.
The ideal candidate will be a qualified pharmacist with strong experience in primary care. Essential qualifications include a minimum of 2 years post-qualifying experience, completion of an undergraduate degree in pharmacy, and working towards an independent prescribing qualification.
