Position Overview
Mid Norfolk Primary Care Network (PCN) is seeking a qualified clinical pharmacist to enhance our dedicated team, providing support across multiple GP practices in the Dereham region.
Key Responsibilities
- Oversee the workflow of prescription requests.
- Conduct comprehensive face-to-face medication assessments, focusing on patients with multiple medications, deprescribing, and opioid management.
- Facilitate network quality initiatives, including the sharing of best practices and conducting cross-network audits.
- Design and implement medicines optimization services within the network's practices.
- Lead efforts in quality enhancement and clinical audits, while managing aspects of medication safety and antibiotic stewardship.
- Assist in the management of patients with chronic conditions.
- Perform clinical medication evaluations with patients and provide recommendations to senior clinical pharmacists, nurses, or GPs regarding prescribing and monitoring.
- Collaborate with care home personnel to enhance the safety of medication ordering and administration.
Candidate Profile
The ideal candidate will possess a robust background in primary care pharmacy. Required qualifications include:
- A minimum of 2 years (or equivalent) post-qualification experience as a pharmacist.
- Completion of an undergraduate degree in pharmacy.
- Progress towards an independent prescribing qualification.
- Exceptional interpersonal, influencing, and negotiation skills.
- Strong written and verbal communication abilities.
- Capability to convey complex and sensitive information clearly to diverse patient populations.
- Comprehensive therapeutic and clinical knowledge, with an understanding of evidence-based healthcare principles.
- Proficient IT skills.
- Adept at planning, managing, advising, and reviewing general medication optimization issues related to long-term conditions.
- Ability to work effectively both independently and as part of a team.
Desirable Qualifications
- Possession of an independent prescribing qualification.
- Clinical Diploma.
