About the Role

Mid Norfolk Primary Care Network (PCN) is seeking an experienced clinical pharmacist to join their team on a full-time basis. The successful candidate will be responsible for managing the prescription request workflow, conducting complex face-to-face medication reviews, and developing and managing medicines optimization services within the network practices.

Main Responsibilities
  • Manage the prescription request workflow to ensure efficient and effective medication management.
  • Conduct complex face-to-face medication reviews with a focus on polypharmacy, deprescribing, and opiate prescribing.
  • Develop and manage medicines optimization services within the network practices to improve patient outcomes.
  • Provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship.
  • Support the management of patients with long-term conditions and undertake clinical medication reviews to produce recommendations for senior clinical pharmacists, nurses, or GPs.
  • Work with care home staff to improve the safety of medicine ordering and administration.
About the Candidate

The ideal candidate will be a qualified pharmacist with strong experience in primary care. Essential qualifications include:

  • Minimum of 2 years (or equivalent) post-qualifying experience as a pharmacist.
  • Completion of an undergraduate degree in pharmacy.
  • Working towards an independent prescribing qualification.
  • Excellent interpersonal, influencing, and negotiating skills.
  • Excellent written and verbal communication skills.
  • Ability to communicate complex and sensitive information in an understandable form to a variety of patients.
  • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
  • Excellent IT skills.
  • Ability to plan, manage, monitor, advise, and review general medicine optimization issues in core areas for long-term conditions.
  • Work effectively independently and as a team member.
