About the Role
Mid Norfolk Primary Care Network (PCN) is seeking an experienced clinical pharmacist to join their team on a full-time basis. The successful candidate will be responsible for managing the prescription request workflow, conducting complex face-to-face medication reviews, and developing and managing medicines optimisation services within the network practices.
Main Responsibilities:
- Manage the prescription request workflow.
- Conduct complex face-to-face medication reviews with an emphasis on seeing patients with polypharmacy, deprescribing, and opiate prescribing.
- Develop and manage medicines optimisation services within the network practices.
- Provide leadership on quality improvement and clinical audit as well as managing aspects of medicines safety and antibiotic stewardship.
- Support the management of patients with long-term conditions.
- Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacist, nurses or GPs on prescribing and monitoring.
About the Candidate
The ideal candidate will be a qualified pharmacist with strong experience within primary care. Essential qualifications include a minimum of 2 years post-qualifying experience as a pharmacist, completion of an undergraduate degree in pharmacy, and working towards an independent prescribing qualification.
Desirable qualifications include holding an independent prescribing qualification and a clinical diploma.
