Clinical Pharmacist

Mid Norfolk Primary Care Network (PCN) is seeking an experienced clinical pharmacist to join their team on a full-time basis. The successful candidate will be responsible for supporting six GP practices across the Dereham area.
Main Responsibilities- Manage the prescription request workflow to ensure efficient and effective medication management.
- Conduct complex face-to-face medication reviews, focusing on patients with polypharmacy, deprescribing, and opiate prescribing.
- Lead network quality activities, including sharing best practice and cross-network audit, to drive excellence in patient care.
- Develop and manage medicines optimisation services within the network practices, promoting evidence-based healthcare.
- Provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship.
- Support the management of patients with long-term conditions, ensuring comprehensive care and treatment.
- Undertake clinical medication reviews with patients, producing recommendations for senior clinical pharmacists, nurses, or GPs on prescribing and monitoring.
- Collaborate with care home staff to improve the safety of medicine ordering and administration.
We are looking for a qualified pharmacist with strong experience in primary care. The ideal candidate will possess:
- A minimum of 2 years (or equivalent) post-qualifying experience as a pharmacist.
- Completion of an undergraduate degree in pharmacy.
- Working towards an independent prescribing qualification.
- Excellent interpersonal, influencing, and negotiating skills.
- Excellent written and verbal communication skills.
- Ability to communicate complex and sensitive information in an understandable form to patients.
- In-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare principles.
- Excellent IT skills.
- Ability to plan, manage, monitor, advise, and review general medicine optimisation issues in core areas for long-term conditions.
- Capacity to work effectively independently and as a team member.
Mid Norfolk PCN consists of six GP surgeries working collaboratively as a group. The successful candidate will be part of this dynamic team, contributing to the delivery of high-quality patient care.
