Main Responsibilities
  • Manage the prescription request workflow to ensure efficient and effective medication management.
  • Conduct complex face-to-face medication reviews with a focus on polypharmacy, deprescribing, and opiate prescribing.
  • Develop and manage medicines optimization services within the network practices to improve patient outcomes.
  • Provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship.
  • Support the management of patients with long-term conditions and undertake clinical medication reviews.
  • Work with care home staff to improve the safety of medicine ordering and administration.
About the Candidate

We are looking for a qualified pharmacist with strong experience in primary care. The ideal candidate will have a minimum of 2 years of post-qualifying experience, an undergraduate degree in pharmacy, and be working towards an independent prescribing qualification.

The successful candidate will possess excellent interpersonal, influencing, and negotiating skills, as well as excellent written and verbal communication skills. They will also have in-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
