Job Overview

We are seeking a skilled Community Learning Disability Nurse to join our team at Oxleas NHS Foundation Trust. This is an exciting opportunity to work in a community setting, providing high-quality healthcare to adults with learning disabilities.

About the Role

This fixed-term contract will cover maternity leave for 12 months, finishing on December 2025. As a Community Nurse in Complex Physical Health, you will be part of a small team supporting people with complex physical health needs. Your primary focus will be to provide holistic care, assessments, and interventions to manage a caseload of adults.

Key Responsibilities
  • Provide evidence-based, skilled nursing care to adults with learning disabilities
  • Facilitate health education and health promotion training and group work as needed
  • Lead on providing robust health liaison to acute and primary care to ensure service users access mainstream health services
  • Work alongside the multidisciplinary team (MDT) and wider services to address issues related to service users' accessing services relevant to their individual health and wellbeing needs
Requirements

To be successful in this role, you must have:

  • At least 3 years post-registration experience of working with people with learning disabilities
  • Experience of working in a community setting
  • Planning and organisational skills
  • Experience of using clinical nursing skills
