About the Role
Southern Health NHS Foundation Trust is seeking an experienced Community Nurse to join our Havant Community Care Team. As a Community Health Specialist, you will be responsible for providing high-quality care to patients with complex needs in their own homes.

Main Responsibilities
As a Community Health Specialist, you will be responsible for managing a defined community caseload, working closely with GPs and other healthcare professionals to ensure seamless care delivery. You will also be responsible for coordinating and delegating work, ensuring that referrals are dealt with according to clinical need and that care plans are recorded in line with Trust and professional standards.

Person Specification
We are looking for an experienced Community Nurse with excellent clinical, managerial, and communication skills. You will have a proactive and positive attitude to learning and be able to work autonomously, supporting the team lead to manage the case load and staff development. Access to a vehicle and possession of a full driving license are essential.
