Job Summary:

We are seeking a highly skilled and dedicated Social Worker to join our Mental Health Team in Hampshire. As a Community Social Care Specialist, you will be responsible for providing innovative, person-centred support to individuals and their families, promoting independent and safe living within the community.

Your Responsibilities:

  • Provide expert social care services to adults in need, focusing on prevention, early intervention, and crisis resolution.
  • Develop and implement tailored plans to promote independence, safety, and well-being.
  • Work collaboratively with healthcare professionals, providers, and individuals to ensure seamless service delivery.
  • Conduct comprehensive assessments, identifying needs and developing effective solutions.
  • Communicate effectively with stakeholders, ensuring clear understanding of service provision and progress.

Requirements:

  • Degree in Social Work or equivalent qualification.
  • Registered with the relevant professional body (HCPC).
  • Excellent communication and interpersonal skills.
  • Ability to work flexibly, including occasional evenings and weekends.
  • Valid driver's licence and access to a reliable vehicle.

What We Offer:

  • A competitive salary range of £30,885 - £36,801 per annum, plus market supplement.
  • A hybrid working arrangement, combining office and home-based work.
