Community Social Care Specialist

Job Summary:
We are seeking a highly skilled and dedicated Social Worker to join our Mental Health Team in Hampshire. As a Community Social Care Specialist, you will be responsible for providing innovative, person-centred support to individuals and their families, promoting independent and safe living within the community.

You will work closely with healthcare professionals, providers, and individuals to encourage proactive and collaborative approaches to care. With a strong focus on strengths-based practice, you will use your excellent relationship management skills to negotiate and collaborate with stakeholders to achieve positive outcomes.
Your Responsibilities:
- Provide expert social care services to adults in need, focusing on prevention, early intervention, and crisis resolution.
- Develop and implement tailored plans to promote independence, safety, and well-being.
- Work collaboratively with healthcare professionals, providers, and individuals to ensure seamless service delivery.
- Conduct comprehensive assessments, identifying needs and developing effective solutions.
- Communicate effectively with stakeholders, ensuring clear understanding of service provision and progress.
Requirements:
- Degree in Social Work or equivalent qualification.
- Registered with the relevant professional body (HCPC).
- Excellent communication and interpersonal skills.
- Ability to work flexibly, including occasional evenings and weekends.
- Valid driver's licence and access to a reliable vehicle.
What We Offer:
- A competitive salary range of £30,885 - £36,801 per annum, plus market supplement.
- A hybrid working arrangement, combining office and home-based work.
