About the Role
Southern Health NHS Foundation Trust is seeking an experienced Nurse to join their Havant Community Care Team. As a Community Health Practitioner, you will provide care to patients with complex care needs, enabling them to receive the care they need in their home.

Main Responsibilities
The post-holder will have responsibility for managing a defined community caseload via Nursing, Physiotherapy, or Occupational Therapy interventions for a group of GP Practices as part of the Integrated Care Team.
The post holder will have responsibility for managing, coordinating, and delegating work, ensuring referrals are dealt with according to clinical need, holistic assessments, and care plans are recorded in line with Trust and Professional record-keeping standards.
The post holder will be required to work with a high degree of autonomy and professional accountability, aiming to prevent unnecessary admissions of patients to an acute hospital and support timely discharge into the community.

Person Specification
The post holder must have relevant experience in healthcare and demonstrate a proactive and positive attitude to learning the skills required to fulfill the role.
Access to a vehicle and possession of a full driving license are essential.
