Job Summary

An exciting opportunity has arisen for a Registered or Newly Qualified Nurse to join Southern Health NHS Foundation Trust's established Mental Health Community OPMH Team. The successful candidate will be responsible for care coordination of service users, duty worker on rotation, initial and follow-up assessments, and supervision and mentorship of junior staff and students.

Main Responsibilities

* To act as a care coordinator to an agreed caseload of service users with severe and enduring mental illness, including organic and functional illnesses.

* To undertake ongoing assessment, treatment, and evaluation for service users known to the service.
