About the Role

Southern Health NHS Foundation Trust is seeking an experienced Nurse to join the Havant Community Care Team. As a Community Health Care Specialist, you will be responsible for providing care to patients with complex care needs, enabling them to receive the care they need in their home.

You will be working in a leading organisation that is committed to improving the quality of community health services across Hampshire. The successful candidate will have excellent clinical, managerial, and communication skills, with the ability to work autonomously and support the team lead to manage the case load and staff development.

The post holder will have responsibility for managing a defined community caseload, via Nursing, Physiotherapy, or Occupational Therapy interventions, for a group of GP Practices as part of the Integrated Care Team. They will be required to work with a high degree of autonomy and professional accountability, with a primary aim of preventing unnecessary admission of patients to an acute hospital and supporting timely discharge into the community.

Main Responsibilities

  • Manage and coordinate work to ensure referrals are dealt with according to clinical need
  • Record holistic assessments and care plans in line with Trust and Professional record keeping standards
  • Ensure clinical care meets acceptable standards of patient safety and quality outcomes
