About the Role

We are seeking a skilled Clinical Care Navigator to join our team at Southern Health NHS Foundation Trust. As a Clinical Care Navigator, you will play a vital role in transforming mental health care by working across a range of clinical and non-clinical services to support the delivery of high-quality care.

Main Responsibilities

Key responsibilities of the role include:

  • Supporting the planning, coordination, and transfer of service users across care pathways
  • Identifying care needs and supporting clinicians to develop effective care plans
  • Assisting patients through clinical and non-clinical pathways, ensuring seamless transitions and continuity of care
  • Working with stakeholders and care teams to enable access to relevant interventions and support
