Havant and Waterlooville Primary Care Network is seeking a highly skilled Clinical PCN Pharmacist to join our team. Job Overview

This role is pivotal in improving the quality of patient care and developing operational efficiencies within general practice. As a Clinical PCN Pharmacist, you will work closely with healthcare professionals to provide structured medication reviews, support patients in managing their long-term conditions, and oversee the management of medicines on transfer of care.

The successful candidate will have excellent communication skills, be able to work effectively as part of a multi-disciplinary team, and possess a strong passion for delivering an excellent service. Key Responsibilities
  • Manage a medicines management plan and deliver patient services as determined by and with the Lead Clinical Pharmacist, practice/PCN policy, and local and national guidance.
  • Work as part of the medicines management team within the PCN/practices and with medicines management colleagues within the ICB to acquire and share expertise and best practice to maximise cost-effective prescribing and improve the quality of patient care.
  • Engage with PCN Clinical Pharmacist colleagues and the CD responsible for Clinical Pharmacy to contribute to the development of the PCN medicines management approach and team.
  • Provide medication review services to patients via clinics in member practices, domiciliary visits, and in residential and nursing homes, either face-to-face or via remote consultation technology and deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.
About the Role

This is a challenging and rewarding role that requires motivation and passion to deliver an excellent service within general practice. The salary for this position is estimated at £65,000 per annum, based on industry standards and market rates.
