About the Role

We are seeking a skilled Clinical Care Navigator to join our team at Southern Health NHS Foundation Trust. As a Clinical Care Navigator, you will play a vital role in transforming mental health care services, working closely with clinicians and care teams to identify care needs and support service users across clinical and non-clinical pathways.

Main Responsibilities

Key responsibilities of the role include:

  • Supporting the planning, coordination, and transfer of service users across care pathways
  • Working with clinicians to identify care needs and develop personalized care plans
  • Assisting patients through clinical and non-clinical pathways, ensuring seamless transitions and effective care coordination
  • Identifying and addressing barriers to health and social care, working collaboratively with stakeholders to resolve these issues
  • Developing the role of Care Navigator to embed the No Wrong Door approach within the system
