About the Role
We are seeking a skilled Clinical Care Navigator to join our team at Southern Health NHS Foundation Trust. As a Clinical Care Navigator, you will play a vital role in supporting the delivery of transforming mental health care across our services.
Key Responsibilities
- Work across a range of clinical and non-clinical services to support the delivery of transforming mental health care.
- Support the planning, co-ordination and transfer of service users across the care pathways.
- Support clinicians to identify care needs and assist patients through the clinical and non-clinical pathways.
- Identify areas where barriers to health and social care exist and work with the team to resolve these.
- Work in a manner which is helpful to the service user, based on principles of recovery, personalisation and choice.
