Adult Clinical Nurse Advisor

We are seeking a highly skilled and experienced Adult Nurse Manager to join our team at Leaders in Care. As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, creating personalized care plans, and working closely with a core team of carers to deliver high-quality care.
Key Responsibilities:- Provide expert clinical guidance and support to carers and patients
- Develop and implement effective care plans that meet the unique needs of each patient
- Collaborate with the multidisciplinary team to ensure seamless care delivery
- Conduct regular care rounds and proactive home visits to ensure patient satisfaction and well-being
- Respond to and investigate clinical complaints in line with policy and risk management
- RGN with an active NMC PIN and up-to-date experience of tracheostomy care and ventilators
- Excellent communication and leadership skills, with the ability to train and support carers
- Full UK driving license and use of a vehicle for work
- A valid NMC Pin
- Competitive salary up to £45,000 per annum
- 2 days working from home per week / 3 days in the community
- Monday to Friday (9-5) hours
- 33 days holiday
