Job Title: Healthcare Coordinator

We are seeking a skilled and compassionate Healthcare Coordinator to join our team at The Lennard Surgery. This is an exciting opportunity to make a difference in the lives of patients by providing exceptional support and guidance.

About the Role

The successful candidate will be responsible for navigating patients through their healthcare journey, ensuring they receive the right care, at the right time, and in the right place. This will involve:

  1. Providing comprehensive support and guidance to patients, including signposting them to appropriate healthcare professionals or services.
  2. Building strong relationships with patients to understand their needs and preferences.
  3. Collaborating with our team of GPs, nurses, and allied professionals to ensure seamless care delivery.
  4. Managing appointments to minimize wait times and maximize efficiency.
Key Responsibilities

The Healthcare Coordinator will be responsible for:

  • Receiving and processing patient requests, including appointment bookings and prescription queries.
  • Responding to incoming phone calls, transferring calls or dealing with caller's requests as necessary.
  • Entering patient data into our clinical system, ensuring accurate and up-to-date records.
  • Maintaining a clean and organized working area, adhering to infection control protocols.
Requirements

To succeed in this role, you will need:

  • Educated to GCSE level or equivalent.
  • Active signposting or Care Navigator training qualification.
  • Healthcare qualification (level 2) or working towards gaining equivalent level.
