Job Summary

We are seeking an experienced and compassionate Senior Clinical Care Coordinator to join our team at one of our care homes in the UK. As a key member of our healthcare team, you will be responsible for coordinating the delivery of high-quality clinical care to our residents.

Key Responsibilities
  • To coordinate the delivery of clinical care to residents, ensuring that their needs are met and exceeded.
  • To work closely with other healthcare professionals to develop and implement individualized care plans.
  • To monitor and evaluate the effectiveness of care plans and make recommendations for improvement.
  • To provide guidance and support to junior staff members and ensure they receive the training and development needed to excel in their roles.

This role requires a high level of empathy, communication skills, and ability to prioritize tasks effectively. You will also need to be flexible and adaptable, as the demands of this role can change quickly.

The successful candidate will have a background in nursing or a related field, with experience in care coordination and leadership. They will be able to demonstrate excellent communication and interpersonal skills, as well as a passion for delivering high-quality patient care.
