Job Title: Patient Care Navigator

Job Summary:

The Lewisham Care Partnership is seeking a highly skilled and compassionate Patient Care Navigator to join our team. As the first point of contact for patients, you will provide exceptional customer service, arrange appointments, and offer guidance on available services and options.

This role will be based in our central call centre and at our practice sites, requiring flexibility to work in both settings. You will be responsible for processing patient requests, answering phone calls, and entering data into our clinical system.

Main Responsibilities:

  • Process and signpost patients to the appropriate healthcare professional
  • Answer phone calls, transfer calls, or handle caller requests
  • Process patient requests for appointments and repeat prescriptions
  • Initiate contact with patients, team members, and external agencies
  • Enter read-code data on Emis Web clinical system
  • Photocopy documentation as required
  • Data entry of new and temporary registrations and relevant patient information
  • Input data into patients' healthcare records
  • Direct requests for information, SAR, insurance/solicitors letters, and DVLA forms to the administrative team
  • Manage queries efficiently and carry out system searches as requested
  • Maintain a clean and tidy working area and monitor the reception area and noticeboards
  • Support clinical staff with general tasks as requested

Person Specification:

  • Desirable qualifications: GCSE level or equivalent, active signposting or Care Navigator training qualification, healthcare qualification (level 2) or working towards gaining equivalent level, customer service qualification (NVQ) or equivalent
  • Desirable experience: working in a primary care environment, working with the general public, working in a healthcare setting
  • Desirable skills: excellent communication skills, strong IT skills, clear telephone manner, competent in Office and Outlook, EMIS / Systmone / Vision user skills, effective time management, ability to work as a team member and autonomously, good interpersonal skills, ability to follow clinical policy and procedure
