About the Role

The Senior Social Worker will be responsible for undertaking comprehensive assessments and developing support plans to promote independence and well-being among older adults and physically disabled individuals in Lancashire.

You will work closely with colleagues and external agencies to provide holistic support services, including conducting home visits and coordinating care packages.

The ideal candidate will have a strong background in social work, excellent communication skills, and the ability to work effectively in a team environment.
