Key Responsibilities

  • Deliver clinical and therapeutic care to patients, working within established protocols and guidelines.
  • Conduct patient assessments, develop and implement individual care plans, and evaluate patient progress.
  • Manage a caseload of non-complex patients, providing support and guidance as needed.
  • Coordinate safe patient admissions, transfers, and discharges, ensuring timely and effective discharge planning.
  • Assist patients with personal care, including oral hygiene, grooming, and dressing needs, while respecting their cultural and religious backgrounds.
  • Use equipment and mobility aids safely and competently, reporting any issues or accidents to the therapy team.

Requirements

  • Significant experience working in a patient-facing environment, with a reablement or rehabilitation focus.
  • Previous experience working with patients with physical, cognitive, or behavioral impairments.
  • Commitment to undertaking a Foundation Degree and associated training.
  • Level 3 Health and Social Care Diploma or equivalent.
  • Ability to work well in a team, independently, and under pressure.
  • Clear and concise communication skills, with the ability to liaise with multidisciplinary teams and patients.
