About the Role
We are seeking a skilled and compassionate Healthcare Assistant Practitioner to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and supporting our patients to achieve their goals.
Key Responsibilities
- Deliver clinical and therapeutic care to patients, working within established protocols and guidelines.
- Conduct patient assessments, develop and implement individual care plans, and evaluate patient progress.
- Manage a caseload of non-complex patients, providing support and guidance as needed.
- Coordinate safe patient admissions, transfers, and discharges, ensuring timely and effective discharge planning.
- Assist patients with personal care, including oral hygiene, grooming, and dressing needs, while respecting their cultural and religious backgrounds.
- Use equipment and mobility aids safely and competently, reporting any issues or accidents to the therapy team.

Requirements
- Significant experience working in a patient-facing environment, with a reablement or rehabilitation focus.
- Previous experience working with patients with physical, cognitive, or behavioral impairments.
- Commitment to undertaking a Foundation Degree and associated training.
- Level 3 Health and Social Care Diploma or equivalent.
- Ability to work well in a team, independently, and under pressure.
- Clear and concise communication skills, with the ability to liaise with multidisciplinary teams and patients.
