About the Role

We are seeking a highly skilled and passionate Occupational Therapist to join our Front Door Team in Adult Social Care. As a key member of our team, you will contribute to a multi-disciplinary team and hold a small, short-term case load for assessment, supporting residents to maximize their strengths, wellbeing and independence.

The Front Door Team is a vital service within Adult Social Care, providing a first point of contact for residents and professionals who need advice and support from a range of services, agencies or community offers. We work closely with partners such as healthcare providers, local authorities, and community organizations to ensure people receive a timely and appropriate response to their enquiry.

Key Responsibilities

As an Occupational Therapist in our Front Door Team, you will be responsible for:

  • Conducting assessments and providing support to residents to maximize their strengths, wellbeing and independence
  • Working collaboratively with a multi-disciplinary team to ensure effective service delivery
  • Developing and maintaining strong relationships with partners and stakeholders to ensure seamless service provision
  • Providing expert advice and guidance to residents, families, and professionals
